fwiw, I have the 2 stroke bike kit from the first post installed on my boat. I haven't used it offshore in heavy seas, but in the bay it works very well. I think it's a 4:1 ratio, and I have it on a wiper motor. If doing it fresh, I might opt for a power steering motor to get extra torque. I first tried a tension pulley approach using the roller supplied with the kit, but I found it difficult to get working as I wanted. I opted for a "hinged box" design instead with a bolt latch, and it suits my needs. As a note, I did have to order a different diameter of hdt5m belt to fit my wheel pedestal installation. The small gear on the wiper motor is secured with a nut, JB Weld, and a set pin through the axle.

One other thought that I tried with limited success: you could try laying a belt teeth-in inside of a styrofoam mold with a circular cutout, then fill the interior with thickened epoxy or poly resin to "cast" a gear.
