As I told you I don't have a Pi4 so it was mainly guessing from online images. I hope it will work, if not let me know and we will try to fix it.

About your other questions: there is no an "original case", you simply choose the modules you need, print them and create the case you need for your specific configuration. 

Suppose you are going to use only a Raspberry Pi4 with a MacArthur HAT, you would need to download 4 files: the base + the Pi 4 spacer + the MacArthur spacer + the lid.

Then pass this four files to somebody with a 3d printer and print them. Put your boards inside, add some screws to hold it togheter and voilá.

I don't know what "OST" means, excuse my ignorance.
